An Online Learning Opportunity for Your Patients

Every minute at least one person in the U.S is diagnosed with heart failure.

Give your patients the tools they need to understand their risk of heart failure and the steps to making small changes that can lead to big results.

We invite your patients to view our 15-minute “Heart Failure: An Overview” video presented by Preeti Kolankari, Senior Manager for Patient Engagement at The American Heart Association’s National Center. She provides details about who is at risk, signs and symptoms to be aware of and how to discuss the topic and concerns with a physician.

Please download the patient education flier provided and give it to patients that you feel would benefit from learning about heart failure.

Your patients’ participation is a simple as visiting our Web site and providing their name, state and age to access the video. There they will view the presentation and find links to additional educational resources about heart failure.
